The Community Residential Alternative Program (CRA) is available to current and potential individuals.
CRA provides 24 hour residential oversight in a licensed Community Living Arrangement home. Services include but are not limited to the following:
- Protective Care and Watchful Oversight
- Self-Administration of Medication including acquiring medication refills and/or obtain new medications 3 balanced meals per day
- 2 nutritious snacks per day
- Bedding and supplies
- Person Centered recreational and cultural activities
- Person Centered Implementation of the Individual Service Plan & Goal tracking
- Supervision of Nutrition
- Medical Appointments
- Emergency/Non-Emergency Transportation
- Arrange for Transfer and/or Discharge when necessary
- Nursing Services
- Continuous assessment of needs and condition
- Informing relatives, the service coordinator/monitor, legal representative, DBHDD regarding any individual’s need with individual’s permission
- Purchase clothing and personal hygiene supplies as needed
- Personal laundry
- Assistance and or training with daily living skills
