The Community Residential Alternative Program (CRA) is available to current and potential individuals.
CRA provides 24-hour residential oversight in a licensed Community Living Arrangement home. Services include, but are not limited to the following:
- Protective Care and Watchful Oversight
- Self-Administration of Medication, including acquiring medication refills and/or obtaining new medications
- 3 balanced meals per day
- 2 nutritious snacks per day
- Bedding and supplies
- Person-centered recreational and cultural activities
- Person-Centered Implementation of the Individual Service Plan & Goal Tracking
- Supervision of Nutrition
- Medical Appointments
- Emergency/Non-Emergency Transportation
- Arrange for Transfer and/or Discharge when necessary
- Nursing Services
- Continuous assessment of needs and conditions
- Informing relatives, the service coordinator/monitor, legal representative, and DBHDD regarding any individual’s needs, with the individual’s permission
- Purchase clothing and personal hygiene supplies as needed
- Personal laundry
- Assistance and/or training with daily living skills
